Baby teeth do big jobs. They help children chew and speak clearly, guide adult teeth into place, and support healthy jaw growth. Some baby teeth are meant to stay until age 12–14. When one is lost too early—because of a cavity, extraction, or an injury—nearby teeth can drift into the empty space. That crowding can block the permanent tooth, lead to misalignment, and make future orthodontic treatment more complicated. A space maintainer can help.
What is a space maintainer?
- A small, custom-made appliance that holds the space where a baby tooth was lost early
- Made from smooth metal or acrylic to fit comfortably
- Worn only until the permanent tooth is ready to come in
Who might need one?
Not every early tooth loss needs a space maintainer. We consider:
- Your child’s age and growth stage
- Which tooth is missing and how soon the adult tooth is expected
- Current crowding or spacing
- Your child’s cavity risk and ability to keep the area clean
Types of space maintainers
- Fixed (cemented in place)
- Band-and-loop: A common choice for a single missing baby molar
- Crown-and-loop: Used when the anchor tooth needs a stainless-steel crown
- Distal shoe: Guides the first permanent molar when it hasn’t erupted yet
- Bilateral options: Lingual holding arch (lower) to hold space when multiple teeth are missing
- Removable (retainer-style)
- Can include an acrylic “tooth” for appearance
- Best for older, cooperative children who can keep it clean and wear it as directed
How placement works
- Gentle exam and X-ray to confirm timing and plan
- Quick measurements or a digital scan for a custom fit
- For fixed maintainers, we cement it in place at a short visit
- Most kids adjust quickly; mild pressure or cheek irritation usually improves within a few days
Care tips at home
- Keep it clean: Brush twice daily and floss; use a floss threader around bands
- Be food smart: Avoid sticky candies (gummies, caramels), very hard foods, and chewing ice
- No picking or tugging: Fingers and tongues can loosen the appliance
- Stay on schedule: Regular checkups so we can monitor eruption and adjust if needed
- Call us if it feels loose, pokes the cheek or tongue, breaks, or causes persistent soreness
How long will my child need it?
- Typically until the permanent tooth begins to erupt into the space
- We’ll track progress with exams and periodic X-rays and remove the maintainer at the right time
Why it’s worth it
- Helps prevent shifting and crowding
- Protects your child’s bite and guides proper alignment
- Can shorten or simplify future orthodontic treatment
